What is Care Management?

Navigating the world of senior services can be overwhelming at times.  Kinsman Care Management, LLC provides the comprehensive management of all aspects of care.  Assistance can range from a one-time care consultation, to temporary help during a time of transition, to on-going full support and/or guardian services.  The Care Management Process includes a detailed Client Assessment, the development of a Plan of Care, Acquisition and Implementation of Services, on-going Monitoring, and Reassessment. 

Kinsman Care Management, LLC seeks to not only establish care for basic day-to-day needs, but also to provide meaning and purpose when life’s challenges get in the way.  We offer professional services to alleviate family stress, help navigate the aging system, and provide unbiased care for adults with cognitive and physical impairments.


Why Do Individuals and Families Need Care Management?

Care management can be elected or court ordered.  Some families find it difficult to manage the care of their loved one from a distance, especially if he or she has complex physical, cognitive or emotional needs.  Some families who live out of state choose care management because they need someone local to oversee the care of their loved one.  

Sometimes family members have differing viewpoints and cannot come to an agreement about care needs.  In this case, it can be easier for a neutral, non-family member to sort through the conflict and propose a care plan in the best interest of the client.  And some people who need care may not have any family members who are capable of managing their care or making decisions on their behalf.

Our staff has an extensive knowledge of senior services and resources in the Denver Area.  We provide a non-biased perspective—the number one concern is the senior’s well-being and safety.  By managing the care for the elder, family can have peace of mind, even if they live many miles away.  When families are frustrated, care managers can help bring clarity out of confusing or overwhelming situations.

Without a care manager, services are often fragmented by “the system,” causing crucial details pertaining to the care needed to be forgotten or ignored.  Care management provides for continuity of care which improves outcomes for the client.  Since care managers have “the big picture” in mind, they can also make recommendations to contain costs and conserve client’s funds.


Though diagnosed with cancer, my mom did great up until about 10 days before she passed away, and then the need for in-home care became sudden and fairly acute. Roseanne Collison met with our family on short notice and more importantly at a place/time that was convenient for all of us. We had a long list of specific needs and a very specific plan for how to make the in-home care progressive as required by my mom’s condition. Roseanne set everything up for us and the home care was perfect. It graduated up in terms of its intensity as it was needed. We believe our mom was very well cared for in her final days because of the attention she received from Roseanne right from the start of the process.
